Due to packages getting lost or misplaced in the Military Post Office mail system, we must have all APO/FPO or other military ship to addresses fill out and sign the following form stating that you understand that we can only guarantee the package will reach your post office location.

After the product is shipped to you, we have no way of guaranteeing that you will receive the package from your post office.

Please follow the below steps to get your order placed with us:

• Place your order online (to obtain your invoice number)
• Print and fill out the form on this page with the required information (all spaces must be completed before we can ship out your product)
• Fax the completed form back to us

Once we have received the fax, we will be able to process your order.
